How to refer to the CDU
The CDU is pleased to take referrals from educators and employers.
Trainees are also welcome to refer themselves to the CDU, as are any doctors or dentists that are not in Health Education Thames Valley (Oxford Deanery) training programmes or posts, for support with either a career dilemma or a performance improvement need.
Please download the appropriate referral form, entering as much information as possible, and return the completed form to: cdu@thamesvalley.hee.nhs.uk
Referral Forms:
- For Educators or Employers who wish to refer a trainee to the CDU, please download and return the Trainee Referral Form
- For Trainees who wish to refer themselves to the CDU, please download and return the the Trainee Self Referral Form
- For doctors or dentists that are not in Health Education Thames Valley (Oxford Deanery) training programmes or posts who wish to obtain support from the CDU, please download and return the the Referral Form for doctors or dentists not in training
Once your form has been received, a senior member of the CDU team will contact you within 10 working days to discuss what form of CDU support may be of help.
